​Here's a breakdown of our process:
1. Eligibility
Screening
Arms Reach, LLC initiates your journey by thoroughly reviewing all relevant referral documents to determine your need for supportive services. In addition to this, we conduct a face-to-face assessment, enabling us to understand your personal interests, motivations, and desires for our services. This level of screening is customized depending on the services requested.
2. Individualized Treatment Plan
As part of our commitment to a personalized approach, all individuals receiving Arms Reach Services will have an Individual Plan (IP) formulated based on their unique needs and assessment results. This plan, which you will review and sign, identifies specific goals, interventions, and expected outcomes to be achieved within a targeted timeframe.
3. Matching
Our matching process ensures you are paired with a staff member based on the assessment results, compatibility of personality, and their ability to meet your needs in a respectful and dignified manner. We believe this alignment is crucial to the success of your therapeutic journey.
4. System Support & Parental Involvement
We advocate for a holistic approach to care, welcoming all supports necessary for the enrichment of our individuals. Recognizing the importance of active participation and involvement, we understand its positive influence on an individual's desire to improve their quality of life.
5. Supervision & Evaluation of Services
We are committed to providing quality care, which includes monthly supervision of all direct care staff by a licensed clinician. This process helps review the current treatment status of the individuals being served and offers the necessary support and guidance to meet your needs. Furthermore, we evaluate the effectiveness of our services by reviewing the Individual Plan every three (3) months at a minimum. This review allows us to monitor your progress, adjust goals and interventions as needed, and formulate the appropriate transition or discharge plan.
6. Service Delivery
At Arms Reach, we believe that your comfort and safety are paramount. Hence, we aim to provide services in an environment where you feel safe, comfortable, hassle-free, and teachable. Our services are Home-Based, School-Based, Community-Based, and Facility-Based. If you cannot come to us, we will come to you. This approach gives your support system and the community the opportunity to get involved and gain a better understanding of your needs.
